Community Transport Services
The South East Volunteers community transport service has been supporting the City of Monash for over 30 years, providing affordable rides for residents who are unable to drive. Our compassionate volunteers assist directly to your door and conduct safety checks if needed.
We offer services Monday to Friday between business hours, with fees based on destination. Our drivers are thoroughly vetted every three years with police and reference checks, ensuring a safe and reliable experience for all clients.
Our transport service and social support programs aim to keep you living at home as independently as possible, and feeling connected to your community. Together we help you to achieve your goals and maintain your independence. Transport may be booked for medical and social appointments, including:
- Hospital Appointments
- Dentist
- Optometrist
- Specialist
- Physiotherapy
- Podiatry
- Blood tests
- X-rays
- Exercise classes
- Hydrotherapy
- Visit a partner or family member in hospital or care
- Senior Citizens’ clubs
- Church groups
- Educational classes
- Day Centre activities
eligibility
To use our program you need to be 65 years of age or over, or under 65 with a disability and do not have transport already included in your NDIS package.
You must be a resident of the City of Monash, Ashburton, Burwood, Burwood East, Vermont South or Forest Hill and live:
- At home or
- In an independent living unit or
- In a serviced apartment that you own or
- Supported residential service
You must be able to independently:
- Get in and out of a car, without being lifted (we will provide balance support)
If you have an existing home care package please talk to your provider about being referred to SEV.
Otherwise if you are aged 65 years or over; you will be supported through the CHSP program and will need to be registered through My Aged Care to be referred to our service (you can still receive our service until you register with My Aged Care). Please advise your assessor that you would like to be referred to South East Volunteers’ Transport service.

If you are under 65 you may be eligible to be funded by the HACC-PYP Program. To find out more please call us and ask for intake and assessment.
Client are required to register with us before they can access our services. All intake appointments are conducted over the phone and can be booked below. Phone intake appointments are available in English, Mandarin and Cantonese.

What suburbs do you service?
Client must reside within the suburbs in the City of Monash (Ashwood, Clayton, Glen Waverley, Hughesdale, Huntingdale, Mount Waverley, Mulgrave, Notting Hill, Oakleigh, Oakleigh East, Wheelers Hill, parts of Chadstone and parts of Oakleigh South) or within the suburbs of Ashburton, Burwood, Burwood East, Vermont South and Forest Hill.
We transport clients to destinations up to our outer boundary which includes surrounding hospitals such as Box Hill, Ringwood, Knox, Dandenong, Moorabbin, Kingston Centre and Malvern. We do not service destinations in Camberwell, the City or the airport.

Pricing
Commonwealth Home Support Programme (CHSP)
Clients over 65 who provide a CHSP Transport – Direct referral from My Aged Care may use our transport service for a small co-contribution fee of $15-20 for a return journey for destinations within our service area.
If you are using our transport service to attend our social support activities, the return fare is just $5.
You can contact My Aged Care on 1800 200 422 to obtain your 13-digit CHSP referral code.
Program for Younger People (PYP)
Clients under 65 who require short term assistance with their transport needs for transport to medical and wellbeing appointments may access our transport service for a small co-contribution fee of $15-20 for a return journey for destinations within our service area.
If you are using our transport service to attend our social support activities, the return fare is just $5.
Home Care Package (HCP) / Support at Home (SAH)
Clients who have a Home Care Package or Support at Home Package may contact their provider to request transport services or make an arrangement with them to use SEV Transport. Our fares are billed directly to the package provider.
Private Client
Clients who are over 65 and do not have a CHSP referral or a HCP/SAH package may register with us as private client. Our fees are currently $45-55 for a return journey for destinations within our service area.
If you are using our transport service to attend our social support activities, as a private client the return fare is just $35.
